Truvada – everything you need to know about the HIV medication

Truvada is a medication used as PrEP (pre-exposure prophylaxis) that has changed the game in the fight against HIV. But what exactly is this drug and how does it work? When is it worth taking? And what should you know before you start?

What is Truvada and what is it used for?

Truvada is an antiviral medication in the form of blue film-coated tablets. No magic, just solid science – it contains two key active ingredients:

  • Emtricitabine (FTC)
  • Tenofovir disoproxil fumarate (TDF)

These ingredients belong to the class of nucleoside reverse transcriptase inhibitors (NRTIs). In simple terms: they block an enzyme that HIV desperately needs to replicate in your body. Without this enzyme, the virus cannot copy its genetic material or infect additional cells.

Two main uses of Truvada:

  1. Treatment of HIV-1 in people already infected

If someone is living with HIV, Truvada is often part of a combination of antiretroviral drugs (HAART therapy). Important: it is never used alone for treatment, always in combination with other medications!

  1. Pre-exposure prophylaxis (PrEP)

This use has made Truvada a key component of HIV prevention. PrEP protects against HIV infection for people who:

  • Have multiple sexual partners
  • Have sex without condoms
  • Are in a relationship with an HIV-positive partner
  • Engage in chemsex (sex under the influence of psychoactive substances)
  • Work in the sex industry
  • Are men who have sex with men (MSM) at high risk

How to take Truvada? Dosage schedules

For HIV treatment:

  • 1 tablet daily at the same time
  • Always combined with other antiretroviral drugs
  • Strictly follow your doctor’s instructions

For prevention (PrEP):

Daily regimen:

  • 1 tablet every day, preferably at the same time
  • Take with a full glass of water
  • Suitable for all groups, regardless of gender or sexual practices

On-demand regimen (2+1+1):

  • 2 tablets 2–24 hours before planned sexual activity
  • 1 tablet 24 hours after the first dose
  • 1 tablet 24 hours after the second dose

NOTE! The on-demand regimen:

  • Works ONLY for anal sex
  • NOT recommended for women or people with a vagina
  • Requires taking all 4 tablets exactly as planned

PrEP effectiveness – impressive numbers

Clinical studies show that Truvada used as PrEP:

  • Reduces the risk of HIV infection by over 90% with daily use
  • Provides 86% protection with the on-demand regimen (for MSM only)
  • Reaches maximum effectiveness after 7 days of consistent use

Importantly, PrEP effectiveness depends mainly on one factor – consistency. The more closely you follow the dosing schedule, the better your protection.

Truvada – key warnings and myths

Myth #1: “Truvada is a condom in a pill”

FALSE! Truvada protects ONLY against HIV. It does not protect against:

  • Syphilis
  • Gonorrhea
  • Chlamydia
  • HPV and herpes
  • Hepatitis A, B, and C

That’s why combining PrEP with condoms is recommended, especially with frequent partner changes.

Myth #2: “You can use PrEP without medical supervision”

ABSOLUTELY NOT! Taking Truvada requires regular testing:

  • HIV test (every 3 months)
  • Kidney function test (before starting and regularly during treatment)
  • Tests for other sexually transmitted infections
  • Regular medical check-ups

Most common side effects of Truvada:

Most people tolerate Truvada very well, but side effects may include:

  • Nausea and stomach issues (usually temporary)
  • Headaches and fatigue
  • Temporary decrease in kidney function (requires monitoring)
  • In rare cases – reduced bone density

Contraindications – when NOT to use Truvada

Truvada is not for everyone. You should not take it if you:

  • Have severe kidney problems
  • Are allergic to any of its ingredients
  • Are already HIV-positive (without being on full antiretroviral therapy)

MOST IMPORTANT: Always get an HIV test before starting PrEP! Taking Truvada with an undiagnosed infection may lead to drug-resistant virus strains.

Where to get Truvada in Poland? Costs and availability

In Poland, Truvada and its generics are available:

  • By prescription after a consultation with an infectious disease specialist
  • Through reimbursed PrEP programs in selected clinics
  • In some LGBTQ+-friendly healthcare centers

Monthly cost:

  • In reimbursed programs: free of charge
  • Private purchase: around 200–600 PLN (depending on the manufacturer)

WARNING: Never buy Truvada from unverified online sources! You risk your health and could receive counterfeit drugs without active ingredients.

How to start PrEP? Step by step

  1. Find a specialist – an infectious disease doctor is the best choice
  2. Do baseline testing:
    • HIV test (4th–5th generation)
    • Kidney function test
    • Tests for other sexually transmitted diseases
  3. Discuss your needs with your doctor – which regimen is best for you?
  4. Start taking the medication – exactly as prescribed
  5. Regular follow-ups – don’t skip check-ups!

Taking PrEP isn’t just about swallowing pills. It’s part of a conscious approach to sexual health, which includes:

  • Regular testing
  • Open communication with partners
